The CHAMPVA process, step by step

Step 1: Check eligibility
Make sure the veteran is rated P&T (or another qualifying situation applies) and you aren’t TRICARE-eligible. Eligibility guide →

Step 2: Gather documents
Copies of any health insurance or Medicare cards (front and back); a marriage or birth certificate (optional, speeds things up); school letters for children 18–23. Checklist →

Step 3: Apply with VA Form 10-10d
Apply online, by mail, or by fax. If you have other insurance, include VA Form 10-7959c. It’s free; nobody should charge you. Forms →

Step 4: Get your welcome letter and card
If approved, VA mails a welcome letter and a CHAMPVA ID card, which can take up to 6 weeks. Prescriptions use a separate OptumRx card. If VA needs more information, respond with the documents it asks for.

Step 5: Set up direct deposit
You’ll need it before VA can pay you back for any claim you file yourself. Claims guide →

Step 6: Find care
There’s no network. Hospitals that accept Medicare must accept CHAMPVA; for individual doctors, ask “Do you accept assignment from CHAMPVA?” Providers → For prescriptions, use an OptumRx network pharmacy or Meds by Mail. Pharmacy →

Step 7: Claims
Usually your provider files. If you paid out of pocket, file VA Form 10-7959a within 1 year with an itemized bill and proof of payment. Processing takes about 90 days, and you’ll receive an Explanation of Benefits (EOB).

Step 8: Keep your info current
Report new or changed insurance (10-7959c), address changes, school status for children, and Medicare enrollment at 65. Out-of-date insurance info is a common cause of denied claims.

Step 9: If you disagree with a decision
You generally have 1 year from the decision to ask for a review. VA Form 10-0998 explains the three options:
- Supplemental Claim: you have new and relevant evidence. Follow the instructions in your decision letter.
- Higher-Level Review: a senior reviewer looks again at the same evidence (VA Form 20-0996).
- Board Appeal: a Veterans Law Judge reviews it (VA Form 10182, sent to the Board of Veterans’ Appeals).
