How to apply for CHAMPVA (VA Form 10-10d)

You apply with VA Form 10-10d, Application for CHAMPVA Benefits. You file it yourself, for free. No one should charge you to apply.
Three ways to apply
- Online through VA.gov (start at the 10-10d page). You can upload supporting documents as part of the application.
- By mail to: VHA Office of Community Care, CHAMPVA Eligibility, PO Box 137, Spring City, PA 19475 (confirm the address on the current form before mailing).
- By fax. The fax number is listed on VA’s CHAMPVA benefits page and the form.
Checklist of supporting documents
Include what applies to you:
- Other health insurance or Medicare: copies of the front and back of each card (and your Part D card if you have one), with VA Form 10-7959c if applying by mail. See Other health insurance.
- Age 65+ and not eligible for Medicare: a “notice of disallowance” from the Social Security Administration.
- Spouses (optional, speeds processing): marriage certificate, civil union certificate, or common-law marriage affidavit.
- Children: birth certificate (optional, speeds processing); adoption papers if adopted; proof of marriage to the child’s parent for stepchildren; a school certification letter for ages 18–23.
- Newborns: get the baby’s Social Security number first, then add the child as the veteran’s dependent through a VA regional office, then apply.
After you apply
If approved, VA mails a welcome letter and a CHAMPVA ID card. VA says the card can take up to 6 weeks to arrive. VA has a page on what to do after applying, including how to send missing information.
